Station Equipment Connect
This service consists of attaching any station equipment to the dial tone line. This service is performed by OTS or the equipment vendor when the dial tone service is first installed. This service is not applicable to subscribers who install their own equipment.
Station Equipment Move
This service consists of detaching any station equipment from one extension line and attaching it to another line. An equipment move is normally performed in conjunction with a dial tone move. The provisions of this service line statement apply only to situations where OTS or a vendor performs the move. The provisions do not apply when subscribers move their own equipment.
Station Equipment Disconnect
This is the detachment of any rented or leased station equipment from the extension line.

Billing Rates
Station Equipment Connect
Actual cost plus a 5% processing charge. The charge for this service does not include the cost of the station equipment. For contract items, this cost may be included in the contract price.
A Support Service fee of 0.5% shall be applied to the monthly cost of this service.
Station Equipment Move
Actual cost plus a 5% processing charge.

Station Equipment Disconnect
There may be a time and materials charge or a contract item charge. Contact OTS Voice Orders for the actual cost.
How To Order
Contact OTS and provide contact information and a description of what is needed. OTS will work with the agency to determine which form(s) the agency should complete and submit.
